Choline is an essential nutrient that serves as a source of methyl groups for many steps in metabolism; the body needs it to synthesize phosphatidylcholine and sphingomyelin, two phospholipids vital for cell membranes, and to produce acetylcholine, a neurotransmitter important for memory, mood, muscle control, and other brain and nervous system functions, as well as for gene expression, cell-membrane signaling, lipid transport and metabolism, and early brain development. According to the NIH Office of Dietary Supplements, choline has been studied for possible roles in cardiovascular and peripheral artery disease, neurological disorders such as cognitive decline and Alzheimer's disease, and nonalcoholic fatty liver disease (NAFLD). The evidence is limited and inconsistent: several large observational studies found no significant association between choline intake and cardiovascular or peripheral artery disease (one analysis even linked higher intake to increased mortality), reviews including a 2015 systematic review and a 2003 Cochrane review found no clear cognitive benefit from choline or lecithin supplements, and while choline deficiency is associated with fat accumulation in the liver, more research is needed to clarify its role in preventing or treating NAFLD. According to the NIH Office of Dietary Supplements, choline is sold as supplements containing choline only, in combination with B-complex vitamins, and in some multivitamin/mineral products, with typical amounts ranging from 10 mg to 250 mg. The forms of choline in dietary supplements include choline bitartrate, phosphatidylcholine, and lecithin. No studies have compared the relative bioavailability of choline from these different forms.
The main dietary sources of choline in the United States are primarily animal-based products that are particularly rich in choline — meat, poultry, fish, dairy products, and eggs — while cruciferous vegetables and certain beans are also rich in choline, and other sources include nuts, seeds, and whole grains. Among the foods listed, beef liver (356 mg per 3 ounces) and eggs (147 mg per large egg) are especially high — per the NIH Office of Dietary Supplements.
Choline deficiency can cause muscle damage, liver damage, and nonalcoholic fatty liver disease (NAFLD), though frank deficiency in healthy men and nonpregnant women is very rare. Groups most likely to have inadequate choline status include pregnant women (about 90%–95% consume less than the Adequate Intake), people with certain genetic alterations (SNPs) that raise their dietary choline needs, and patients on long-term total parenteral nutrition — per the NIH Office of Dietary Supplements.
High intakes of choline are associated with a fishy body odor, vomiting, excessive sweating and salivation, hypotension, and liver toxicity. The FNB established Tolerable Upper Intake Levels (ULs) — based on amounts linked to hypotension and fishy body odor — of 1,000 mg/day for ages 1–8, 2,000 mg for ages 9–13, 3,000 mg for ages 14–18, and 3,500 mg/day for adults 19 and older — per the NIH Office of Dietary Supplements.
