Red yeast rice is produced by fermenting the mold Monascus purpureus on rice, a process that can produce monacolins including monacolin K, which is structurally identical to the medicine lovastatin. NCCIH states that products containing substantial amounts of monacolin K may effectively lower blood cholesterol, blood glucose, and blood pressure, and may reduce cardiovascular risk in people with metabolic syndrome — but that the FDA considers such products to be unapproved drugs, which cannot legally be sold as dietary supplements in the United States. NCCIH reports that the amount of monacolin K in red yeast rice products varies widely: a 2017 analysis found it ranged more than 60-fold, from 0.09 to 5.48 mg per 1,200 mg, and most labels do not state the monacolin K content. Because monacolin K is chemically the same as lovastatin, products can cause the same side effects as statin drugs, including muscle, kidney, and liver damage, as well as digestive complaints, and they can interact with statin medications. Contamination is also a concern: a 2021 analysis found only one of 37 products was below European safety limits for the toxin citrinin, and four products labeled citrinin-free were in fact contaminated.
