No established upper limit — excess is excreted. Supplements commonly use far more than the DV. General FDA/NIH adult guidance — not medical advice.
Vitamin B12 is a water-soluble vitamin that the NIH says is required for the development, myelination, and function of the central nervous system, for healthy red blood cell formation, and for DNA synthesis. It works as a cofactor for two enzymes — methionine synthase, which converts homocysteine to the amino acid methionine, and L-methylmalonyl-CoA mutase, which converts L-methylmalonyl-CoA to succinyl-CoA in the metabolism of propionate (a short-chain fatty acid). The fact sheet notes that researchers have looked at vitamin B12 in relation to cancer, cardiovascular disease, dementia and cognitive function, and energy and endurance, but it describes the evidence as mixed or inconclusive — for example, the evidence on vitamin B12 and cancer risk points in conflicting directions. More research is needed to clarify these associations — per the NIH Office of Dietary Supplements; not medical advice.
According to the NIH, methylcobalamin and 5-deoxyadenosylcobalamin are the metabolically active forms of vitamin B12, while hydroxycobalamin and cyanocobalamin become biologically active only after the body converts them to those active forms. In dietary supplements the most common form is cyanocobalamin, with adenosylcobalamin, methylcobalamin, and hydroxycobalamin also used; the fact sheet states that no evidence indicates absorption rates differ by form, and that absorption is about 50% at doses below the intrinsic-factor binding capacity (less than 1–2 mcg) but substantially lower at higher doses (about 2% at 500 mcg). Vitamin B12 is also sold in sublingual tablets or lozenges, and the sheet notes evidence suggests no difference in efficacy between oral and sublingual forms.
Vitamin B12 is present naturally in foods of animal origin, including fish, meat, poultry, eggs, and dairy products. The richest sources listed are beef liver and clams; oysters and fortified nutritional yeasts are also very high. Plant foods do not naturally contain vitamin B12, but fortified breakfast cereals and fortified nutritional yeasts are readily available sources with high bioavailability — per the NIH Office of Dietary Supplements.
Groups most likely to be deficient include older adults (especially those with atrophic gastritis), people with pernicious anemia, people with gastrointestinal disorders such as celiac or Crohn's disease, those who have had gastrointestinal surgery, people who follow vegetarian or vegan diets, and the breastfed infants of women who follow vegan diets. Effects can include megaloblastic anemia, fatigue, glossitis of the tongue, palpitations, low blood cell counts, and neurological changes such as numbness and tingling in the hands and feet, with early intervention important to avoid irreversible damage. Because the body stores about 1 to 5 mg of vitamin B12, symptoms can take several years to appear — per the NIH Office of Dietary Supplements.

The for adults 19+ is 2.4 mcg daily; 2.6 mcg in pregnancy and 2.8 mcg during lactation. Adolescents 14-18 also need 2.4 mcg. Younger children need 0.9-1.8 mcg. Infants have an Adequate Intake of 0.4 mcg (0-6 months) and 0.5 mcg (7-12 months). — per the NIH Office of Dietary Supplements; general information, not medical advice.
The Food and Nutrition Board did not establish a (UL) for vitamin B12 because of its low potential for toxicity. Even at large doses, vitamin B12 is generally considered safe because the body does not store excess amounts. — per the NIH Office of Dietary Supplements; general information, not medical advice.

Yes. Gastric acid inhibitors (proton pump inhibitors like omeprazole and lansoprazole, and H2 antagonists like cimetidine and ranitidine) can interfere with vitamin B12 absorption from food and lead to deficiency. Metformin might reduce vitamin B12 absorption and significantly lower serum concentrations. — per the NIH Office of Dietary Supplements; general information, not medical advice.
